Abstract

Wastewater Treatment Plants (WWTPs) play an important role in environmental pollution control resulting from industrial activities. PT XYZ, as one of the tire manufacturing industries, has established a WWTP to treat industrial wastewater. The construction of the latest WWTP unit requires the identification of potential environmental risks due to various potential failure modes that may affect treatment performance and potentially lead to non-compliance with environmental quality standards. This study aims to identify potential risks, determine risk priority levels, analyze root causes, and formulate effective improvement recommendations. The Failure Mode and Effects Analysis (FMEA) method was used to identify and prioritize risks based on the Risk Priority Number (RPN), which is determined from the Severity, Occurrence, and Detection ratings. Following the identification of priority risks, Fault Tree Analysis (FTA) was employed to analyze their root causes. Data were obtained through field observations, document reviews, interviews with WWTP supervisors, WWTP operators, and Environment, Health, and Safety (EHS) personnel, as well as wastewater quality and WWTP operational data. The results indicate that the WWTP effluent still poses a risk of non-compliance with environmental quality standards, particularly for iron (Fe), pH, and BOD parameters. Interview data identified several major potential failure modes, including inadequate aeration, ineffective sedimentation, clogged filters, membrane fouling, chemical dosing errors, pump failures, disinfection process failures, and WWTP overflow. The FMEA results identified the highest-priority risks in the aeration, filtration, and disinfection units. The FTA root cause analysis of the highest-priority risks indicated that the failures were primarily caused by technical, operational, and managerial factors, including human error, inadequate maintenance, suboptimal system design, insufficient SOPs, and the lack of routine audits.
