Abstract

This study aims to analyze the failure modes that occur in the Komori Enthrone E-29 offset printing machine at Printing Company X using the Failure Mode and Effects Analysis (FMEA) and Fault Tree Analysis (FTA) approaches. The ghosting problem that appears in the print results indicates mechanical and operational disturbances that need to be identified systematically. The FMEA results show that the failure mode with the highest RPN value comes from ink roller wear (RPN = 216), followed by clogging in the grease pump and blanket wear. The FTA diagram describes ghosting as an effect of uneven ink distribution, unstable paper pressure, and set-off due to component failure or setup errors. The conclusion of this study shows that the FMEA and FTA approaches are effective in identifying the main causes of failure and can be used as a basis for preventive and risk-based machine maintenance decision making.
